Once you have a product voucher (available from an Affiliate of the Initial Life Support Foundation) you may order by completing the voucher and submitting it with payment via check or money order to the Initial Life Support Foundation. A physician's prescription (if required) and the AED Program Checklist to be completed by the Affiliate must also be submitted before the order can be fulfilled.
